Indian Health Service Nurse (Clinical Care Coordinator) in Whiteriver, Arizona

Summary

This position is located in the Division of Nursing, Outpatient Departments, Whiteriver Service Unit, Whiteriver, AZ, which also includes positions at Cibecue Clinic, Cibecue, AZ. The primary purpose of this position is to provide case management and clinical coordination for patients in the Outpatient

Department. This position reports to Supervisory Clinical Nurse.

Responsibilities

Coordinates appointments and procedures for patients who need further care or continuity of care at other facilities. Develops and maintains iCare patient panels for patients with chronic disease. Coordinates with Outpatient DME coordinator to obtain equipment and needed supplies for patients, works with allied medical services or outside vendors as needed. Assists with arranging appointments for patients within the IHS and at contract facilities. Performs comprehensive professional nursing duties for the care of patients with a variety of injuries and illnesses in the outpatient department. Performs a wide range of nursing care for patients in all age groups and their families who present an unlimited range of different health related problems.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S):

Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. OR Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-09 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Examples include: Management of nursing care in outpatient primary care environments and emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor and evaluate health care options and services which are consistent with patient-centered medical home principles. Experience in coordination of patient care and resources for a caseload of patients across the continuum of care and through care transitions; able to provide patient and family education pertinent to a wide range of conditions and health factors; skilled in supporting patient self-management. You must meet all qualification requirements within 30 days of the closing date of the announcement.

Additional selection(s) of candidates may be possible within 90 days from the date the certificate of eligibles was issued for this announcement. Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. IHS may offer newly-appointed Federal employees credit for their directly related previous non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which they earn annual leave is earned. Works on regularly assigned, rotational or call back basis to provide continuity of nursing care during, evening, nights, weekends, and holidays.

Subject to shift rotation to inpatient areas as needed to provide adequate staffing. This position is covered by a Bargaining Unit. Recruitment and Relocation and/or Permanent Change of Station may be authorized and is subject to availability and service unit approval. If offered, you will be required to sign a one (1) year service agreement. Failure to fulfill the service agreement will result in repayment of the incentive.
