EE is responsible for premium for their spouse or children. * Offered an affordable plan for dental and vision – premiums, if elected, are paid by EE. * 401K plan with ER matching of EE contribution after one year of full time service. * Life insurance of 50K paid by ER. EE may increase the amount of coverage at the EE expense. * Accept visits up to a minimum of 30 billable visits per week in the BrightStar Territory. * Company Vehicle to complete assignments.  * In some cases, the Owner may allow one rural visit (definition of “rural” varies) to count as one and a half or two visits. * After minimum requirements per week, additional visits (afterhours/weekend) paid at “per visit” rate. * Declining visits before reaching the 30 visit/week minimum, negates the guaranteed salary above and reverts to per visit pay. * All time not completing client home health visits you will be in office. * Paid Time off (PTO) will accrue at 0.02 hours/week (1 week per year) after required probationary period. * Probationary period is 90 days - may be extended if employee not able to meet job requirements during that time. * Completes all required documentation and follows policy on timely submission of documentation. Late submission of documentation results in pay reduction as described in orientation. * ER may dissolve this agreement at any time. * EE may dissolve this agreement with a two-week notice in order to prevent lapse in client care. * Presents in a professional manner, wearing BrightStar nametag and scrubs during all visits. * Opens new non-skilled cases and gives report to assigned caregivers or nurses to follow. Responsibilities * Provide thoughtful care to client & patients based on personalized Plan of Care * Monitor and track changes in the client/patient’s conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed * Proactively updates client/patients’ family on plan of care, health changes, and concerns * Directs, initiates, and implements patient care plan * Explains procedures and treatments to patient to gain cooperation, understanding and allay apprehension * Administers prescribed medications and treatments in accordance with approved nursing techniques * Prepares equipment and aids physician during treatment and examination of patient when applicable * Maintains awareness of comfort and safety needs of patient * Observes patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments and significant incidents * Takes and records patients’ vital signs to detect deviations from normal and assesses condition of patient * Responds to life saving situations based upon nursing standards, policies, procedures and protocol * Documents nursing history, physical assessment any education provided for assigned patients * Initiates a patient education plan according to the individualized needs of the patient as prescribed by the physician and/or hospital policy including patient and family instruction Requirements * Current unrestricted RN license in state for which care is being provided * Certified CPR and negative TB skin test or chest x-ray * Finger Print Clearance Card * Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility, preferred. * Licensed driver with valid automobile insurance * Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.  Must be able to lift up to 50 pounds Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. * Employee must be able to perform the essential functions of this job, such as physical requirements including (but not limited to) climbing stairs, the ability to lift/transfer patients and drive. * Successful candidate must submit to post offer, pre-employment physical examination/medical history check, drug screening and background check. * May be required to submit to annual TB and rubella screening. Ability to safely lift, and physically manipulate patients. * Work is normally performed in a typical interior/office/home environment * Moderate physical activity, occasional lifting up to 25lb, standing or walking for more than 4 hours per day * Home environment involves possible exposure to potentially dangerous materials and situations that require following safety precautions and may include use of protective equipment.
