Community Health Systems Bariatric Surgery Coordinator/Center of Excellence - FT - Days in TUCSON, Arizona
This Bariatric Surgery - Center of Excellence Coordinator is a FT days position working as a team member in our dynamic Quality Department.
Coordinates and leads all activities for Service Line programs using an interdisciplinary approach. Responsibility for the operational and quality outcomes of service line for the defined population.
Assesses and monitors the continuity of care for the defined patient population including but not limited to coordination of daily rounds and multi-disciplinary rounds when indicated. Expedites and coordinates the delivery of services to facilitate a patient's progress through the continuum of care, from point of first contact, through the hospitalization period and post discharge to achieve desired outcomes.
Assists in the development of patient care protocols and in updating patient care procedures for Service Line patients based on current literature/trends and evidence-based practice. Functions as a clinical liaison and resource for the hospital program including serving as a liaison between physician/physicians offices.

- Associates or better in Nursing
Licenses & Certifications
- C-AHA BLS Basic Cardiac
- Three (3) years experience in clinical setting specific to defined patient population strongly preferred. Experience with program development, third party payers and marketing preferred.
