As the Senior Manager, you will provide managerial and administrative support to patients and staff. This position will support patients in their successful transitions of care in order to improve quality outcomes and patient/family experience. In this role, you would demonstrate care coordination expertise and oversees the administrative, financial, and personnel management of the Care Coordination post-acute department. This position is responsible for supervising employees and also participates in the selection, orientation and evaluation and of staff.

Schedule: Monday – Friday, 8am-4:30pm

POSITION SUMMARY

This position provides managerial and administrative support to patients and staff. Uses depth and breadth of care coordination knowledge to ensure transition of patients across the continuum of care resulting in increased system alignment and improved quality outcomes, patient/family experience, and system financial outcomes. Demonstrates care coordination expertise and oversees the leadership of administrative, financial, and personnel management of the Care Coordination post-acute department. This position supervises employees and participates in selection, orientation, counseling, evaluation and staff scheduling. Maintains an up to date knowledge of the post-acute service providers in the corresponding communities and has regulatory knowledge of post-acute admission criteria to ensure appropriate outcomes related to delivery of Care Coordination services.

CORE FUNCTIONS

  1. Builds, supports, and maintains effective relationships with internal and external stakeholders and organizations. Represents Banner in an internal/external facing leadership role. Coordinates activities, reviews work, exchanges information, and/or resolves problems in a timely and professional manner.

  2. Responsible for operationalizing post-acute alignment strategies, processes, tasks and auditing compliance of data entry and tracking. Submits required reports and recommendations to leadership regarding operations, programs and services.

  3. Oversees Care Coordination personnel as assigned. Oversees development for department staff as evidenced by continuing educational opportunities, case study reviews, and mentoring. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Works collaboratively with the licensed Care Coordination team to provide feedback and strategize to ensure effective and safe patient discharges.

  4. Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations. This position also participates in the development of Care Coordination goals and objectives in accordance with company standards. Ensures all goals and objectives are met timely and effectively. 5. Serves as a resource and provides leadership assistance to achieve outcomes supportive of the organization’s strategic plan, as well as optimal operational, financial, and patient/family experience outcomes.

  5. Supports and facilitates innovation and change management. Participates in the development, implementation and evaluation of the goals/objectives and process improvement activities within the department and organization. Analyzes data and healthcare trends to gain efficiencies and improve patient outcomes.

  6. Participates in the development of the department budget in conjunction with established goals and objectives. This position is accountable for meeting annual budgetary goals.

  7. Facilitates communication between members of the healthcare team to promote coordination of care to ensure a positive patient/family experience. Oversees facility patient/family experience results specific to transitional care activities and coaches and manages performance improvement activities to maintain positive patient/family experience.

  8. This position is facility or multi-facility based, providing leadership within post-acute care coordination to ensure quality outcomes and resource efficiency. Internal customers: All levels of nursing leadership and staff and other members of the integrated health care team, for the purpose of integrating services, improving patient care, ensuring effective communication systems and facilitating decision making. In addition, supports the identification of opportunities/gaps for development of additional services and/or partnership opportunities for the Post-Acute division. External customers: Patients and families regarding patient/family experience; physicians regarding patient care and program development; agency vendors and contracted services; staff from other health care agencies/providers and community/professional organizations for the purpose of exchanging patient and program information and insurance payers; and the Joint Commission, state, federal and community agencies regarding compliance with laws and regulations.

MINIMUM QUALIFICATIONS

Requires a level of business and clinical knowledge as normally obtained through the completion of a Bachelor’s Degree in business administration, health services administration or related field.

Requires two years of experience in an acute or post-acute care setting. Knowledge of healthcare management as typically obtained through two years of progressive experience in healthcare operations. Knowledge of the requirements of the various regulatory and accrediting agencies as applicable. Requires critical thinking, communication, influence, decision-making, analytical, and flexibility skills to make optimal decisions based on multiple variables and desired outcomes. Must have the ability to practice skills according to department and professional standards and quality requirements. BLS required.
