JFCS Administration High Needs Case Manager II (W. Phoenix) in Phoenix, Arizona

W. Phoenix Location

Critical Tasks

  • Manages a caseload not to exceed a ratio of 1 High Needs Case Manager to 20 Clients

  • Develops and supports the implementation of crisis and safety plans as clinically indicated

  • Monitors the delivery of services identified in client ISP to ensure the integrity and effectiveness of the ISP

  • Recognizes and seeks clinical consultation when clinically appropriate

  • Documents service plans, progress notes and all agency required forms in a timely manner into the electronic medical record system (NextGen) to meet appropriate timeframes

  • Follows HNCM’s standards as defined by the agency for client contact

  • Obtains training and becomes credentialed to complete required Comprehensive Assessments

  • Remains cognizant of the cultural preferences of the client and family

  • Is organized and maintains the ability to communicate, coordinate and connect with all Child and Family Team/Clinical Team participants

  • Serves in the capacity of a Peer Mentor allowing staff to shadow their appointments

  • Is assigned a point of contact for new hires

  • Performs other tasks as required

Key Performance Indicators

  • Working knowledge of all AHCCCS Practice Protocols for serving children and families and adults in a clinical setting

  • Knowledge of primary child/adolescent psychiatric disorders as described in the IDC 9 and DSM IV

  • Knowledge of techniques and approaches to identify the child’s/family’s, adult’s underlying, unmet needs and can differentiate needs from services

  • Familiarity with P.M Section 5.1 Notice Requirements and Appeal Process for Title XIX and Title XXI Eligible persons; and P.M. Section 5.2 Member Complaints

  • Ability to work comfortably in a clinical team setting with the family/client’s home and community

  • Knowledge of T/RBHA’s barrier resolution process

  • Familiarity with RBHA/Health Plan specific processes for P.M. Section 3.17 (Transition of Persons and Practice Protocol on Transitioning to Adult Services)

Metrics

  • Consistently manages the required caseload and maintains required productivity standards

  • Accurately enters documentation into the EHR and accurately bills for services

  • Performs Comprehensive Assessments according to agency requirements

Qualifications

  • Bachelor’s Degree in behavioral health or related field

  • Experience with behavioral health and integrated health that includes coordination of behavioral health and primary care services with care providers

  • Experience with medical social work preferred

  • Bi-lingual in Spanish preferred

  • Must be able to perform the essential functions of the job including prolonged sitting and in certain positions driving, lifting 20lbs, walking, standing and executing dexterity with a keyboard or smart pad.
