HS Diploma/GED

1-2 Years of Experience in Healthcare Environment

Preferred Experience with Electronic Health Record Systems .

The Families F.I.R.S.T.Referral Coordinator will be responsible for processing, monitoring and recording all Arizona FAMILIES F.I.R.S.T (Families in Recovery Succeeding Together) substance abuse referrals, troubleshooting missing information, checking all Regional Behavioral Health Authority (RBHA) enrollment and Medicaid health insurance status (Title 19 of the Social Security Act) on all new and existing FAMILIES F.I.R.S.T clients.

Duties include:

  • Screening the received AFF referral for program appropriateness.

  • Responsible for receiving, auditing, recording, and accurately processing all new AFF referrals received from the Department of Child Safety (DCS) and Jobs program.

  • Available to receive AFF referral requests Monday through Friday 8:00am – 5:00pm except on legal holidays recognized by the state of Arizona.

  • Required to acknowledge receipt of referral, via email to DCS or Jobs Program, within two (2) working hours of the time of receipt.

  • Accountable for notifying DCS / Jobs case manager of any missing information / documentation, including missing guardianship information related to minor children before a referral can be successfully processed.

  • Conducting two (2) follow up attempts to obtain incomplete and/or missing documents from referrals within required timeframes.

  • Accountable for conducting an initial follow up attempt on missing/incomplete documentation within seven (7) business days of receipt of the referral. A second follow up attempt on missing/incomplete documentation is to be completed within an additional seven (7) business days. If no response is received after the two (2) follow up attempts, the referral will be rejected.

  • Submitting service extensions to DCS thirty (30) days before service request expiration.

  • Accountable for creating client chart in the Electronic Health Record (EHR) system, ensuring accuracy of client demographic information, program detail, internal and external contact information, insurance eligibility and payer information.

  • Accurately uploading all required referral documentation into the client Electronic Health Record (EHR) into the appropriate categories.

  • Accountable for required timely notification to the Families F.I.R.S.T. Outreach Department of completed referrals via the Electronic Health Record (EHR) tasking process.

  • Updating the Families F.I.R.S.T. client list with any additional new referral information not received with the initial AFF referral on a daily basis.

  • Responsible for ensuring that all monthly referrals are processed by the 1st of the following month to ensure accurate and timely billing/invoicing.

  • HS Diploma/GED

  • At least 1-2 years of experience in healthcare environment.

  • Must possess effective oral and written communication skills.

  • Preferred - High level of competency in Microsoft Word, Intermediate Level Excel, PowerPoint, Outlook, electronic medical record, NextGen.

  • Must have valid Arizona driver’s license, be 22 years of age with minimum 3 years driving experience and meet requirements of Terros Health’s driving policy.

  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.

  • Must pass a TB Test.
