Responsibilities

The Patient Logistics RN coordinates the transfer center activities for Dignity Health facilities in collaboration with others to include, hospital administration, nursing leadership, medical staff, physicians and transportation vendors. The Patient Logistics RN functions as the liaison between referring hospitals into and between Dignity Health facilities to coordinate the transfer and admission of patients to hospitals. Participates in all activities, meetings, calls, and education to facilitate transfers from all points of entry to the acute hospital setting. The Patient Logistics RN bases facility designation on diagnoses, planned procedure and level of care requirements and acuity.

Qualifications

Minimum

  • ADN, Diploma or foreign equivalent.

  • RN with 3 years of clinical experience in hospital setting with at least one year to include emergency and/or critical care setting.

  • Competent in select psychomotor skills Knowledge of current nursing technique, principles and practice Working knowledge of computer programs such as word, excel and outlook.

Preferred

  • Bachelors level degree in nursing or healthcare related field

  • Background in Utilization Management and a working knowledge of health care reimbursement Background in Neurology/Neurosurgical and Cardiology Experience using electronic bed management systems, preferably TeleTracking Experience in Patient Flow management
